REGISTRATION

For a proper registration process, please read this page carefully before filling out the form.

1. Download your Camper's two-page Registration Form for convenient mail-in.
2. Complete the two pages in their entirety.
3. Attach the most recent copy of your camper's immunization record.
4. Attach the non-refundable $75 Registration Fee per camper.
5. Enclose the tuition for the first week your child will attend mail the form to:

We cannot accept any missing or incomplete forms or payments.

TUITION

We have kept the tuition rates at a minimum in order to help our working parents.

The Camp day goes from 9:00 AM (please be on time) until 4:00 PM. Drop-off is from 7:00 AM to 9:00 AM.

Tuition (per week for five-5 days)

One child..................... $180.00
Two Children................ $285.00 (SIBLINGS ONLY)
Three Children ............. $365.00 (SIBLINGS ONLY)

Tuition(per week for 3 days)

One child..................... $130.00
Two Children................ $235.00 (SIBLINGS ONLY)
Three Children ............. $315.00 (SIBLINGS ONLY)

Tuition payments can be made at the Camp office or through an emailed invoice, therefore, your email address must be filled out on the registration form. We accept Visa, Amex, and Mastercard. If paying by check please mark the memo line with your child's name and the week for which you are paying. Cash is also accepted.

Your registration will not be processed if:

• there is no immunization record
• the first tuition payment is not included with the registration form
• there is no registration fee

PAYMENT DUE DATES

In order to tend to all our camper's needs which include:

• meals
• staffing
• activities
• health

it is necessary to make tuition payments in a timely manner. Please pay particular attention to the following payment due dates:

Week 1: June 20—June 24, 2011	June 1st
Week 2: June 27—July 1, 2011	June 1st
Week 3: July 4—July 8, 2011 (Camp Closed 7/4)	June 1st
Week 4:July 11—July 15, 2011	June 20th
Week 5:July 18—July 22, 2011	June 20th
Week 6:July 25—July 29, 2011	July 7th
Week 7:August 1—August 5, 2011	July 7th

DATES FOR ATTENDANCE

Note that the days for attendace have the options of 3 or 5 days only. Please indicate which option by circling it on the registration form.

MEDICAL EXAMINATIONS

Medical examinations are not required. However, the medical form, completed and signed by a parent or guardian, must be submitted to the camp along with your child's registration. Please be sure to note any allergies. All medications-presctiptions and otherwise-must be in their original containers and accompanied by a physician's note, and a note of authorization from the parent. Medicines should be left in the camp office at check-in time. PLEASE do not send them in with your child.
